CQC Provider Application Requirements

In order to expedite your CQC provider application, please provide the following information below?

1] Full Name of Organisation and logo
2] Address, Email, Website Address, telephone of organisation
3] Provide a description of your business location (accessibility, equipment, resources, etc. Is this a commercial office or home one? Do you have permission from the landlord or mortgage provider (if applicable)?
4] Name of director/s (their date of birth, email address and telephone number)
5] Name of Nominated Individual (DBS Number and date issued, date of birth, email, address and telephone number)
6] Name of Registered Manager (Date of Birth, telephone, email address)
7] Which age groups do you want to provide services to?                                                                                                           8] Which service bands do you want to offer (personal care, dementia, mental health, autism/learning disabilities, sensory, etc)?                                                                                                                      9] Insurance Quotation                                                                                                                        10] ICO Certificate                                                                                                                                11] Completed Financial Viability Form                                                                                               12] Information about any enquiries with local authorities and the NHS as part of researching the viability of your services.

Evidence 9, 10 and 11 will be need to be submitted with the application as part of the evidence bundle. 

Evidence 12 is usually required at a later stage when the CQC has asked you to complete the application form for 'personal care providers'.
