Please ensure you let us know of any issues that can potentially compromise your integrity as a fit person, such as suspensions. You are also expected to demonstrate your leadership and management skills and knowledge and understanding of the regulated activity you want to manage. In order to fully complete your registered managers' application, please ensure you provide the following information to the consultant:
1] Your full name and date of birth.
2] Your full working history from the age of 16. There should not be any gaps of more than 28 days. Any gaps will need to be explained.
3] Ensure you highlight the start and end dates of each employment period (dd/mm/yyyy).
4] You also need to outline your job role, duties and the reason you left as part of the work history.
5] Outline your qualifications and experience when it comes to the regulated activity you want to provide. There should be reference to leadership and management.
6] You can also provide your professional body registration if you are a GP, Nurse, Social worker, etc
7] Provide your DBS number and the date it was issued.
8] Provide the name of your GP, Surgery Name, address, including their contact number.
9] Provide a reference. The information should include the person's name, email address, telephone number, and address.
