You are responsible for checking that the application reflects your needs and service. You will also need to submit the application using your own email. Your application will need to be emailed to:HSCA_Applications@cqc.org.uk
An application is only complete when both the provider and registered managers' forms, along with your supporting evidence, is submitted under the same application reference.
You can consider using the following words to send an email and submit the application to the CQC.

Please note that from July 2025, the CQC no longer require evidence of financial viability and ICO certificates for domiciliary providers on application.
